Oh, how awful for Sam. His joy at feeling pain after being in the void - horrible, but wonderfully described. You got the brothers' forgetfulness of Adam in there, too. You're so clever.
Also, Michael's problem summed up in two short sentences: "In truth, I'm not sure. It's not something I need to know." He still hasn't learned the lesson his Father was hoping for.
I think the casual feel of all the horrible things happening is what made it really creepy. That, and Michael's smile and 'oops' as he destroyed Sam.
Then, to end with a bored Lucifer. *shudders* You know that isn't going to go well.
